When we met Ms. Barbara back in November, her goal was to spend more time with her grandchildren, ages nine and eleven.
This was incredibly difficult, as she was unstably housed and managing a host of complex medical and behavioral health concerns. She was so busy staying on top of her health that she didn't have the energy to explore independent housing or food assistance programs on her own.
After completing a medication reconciliation, PCIC care coordinators worked with Ms. Barbara to help her understand and make the most of her Medicare benefits. We also connected her to local agencies that were able to provide food assistance and locate stable housing.
In Ms. Barbara's words, "My life was a roller coaster when you all came in and you helped keep things calm. I want to tell your other PCIC clients to really use these services while you have them."
Ms. Barbara is doing very well now – she transitioned from a volunteer position into a paid employee at a local hospital providing compassion and warmth during the check in process. As a cancer survivor herself, she takes tremendous pride in her work helping others feel comfortable in the hospital.
Her new apartment complex has a pool, and she enjoys hosting her grandchildren to play in it on the weekends. Spending time for them is good for her mental health.
We are excited that Ms. Barbara graduated from our Care Coordination program with more stability in her life – she is a strong advocate for herself and we are proud of the work she has done to reach her health and lifestyle goals.
