No way! Here’s why.

It’s been long understood that social factors such as housing, food, transportation, education, income, wealth, zip code, and race are fundamental determinants of one's health. Currently, both insurers and providers are working hard to examine what they can do to address the ever-presence of social gaps that have historically contributed greatly to health inequity.
All interventions to address these social challenges must be unbiased and include repeating rounds of analysis. There needs to be engagement from the community and crucial stakeholders when addressing and grappling with social challenges. Empirical and robust data is now available to assist in making decisions about programs and initiatives that will change how care is delivered.
Through PCIC’s Community Care Coordination program we are leaning in to learn from our clients about the hurdles they face in striving for wellness. This is painting a clear picture of the social determinants at play in their lives and the communities where they live. By addressing Social Determinants of Health thoughtfully, quickly, and more effectively we will keep this from being a healthcare fad.
