Harris Health’s Multi Visit Patient Initiative has had a flourishing relationship with PCIC for years now. Through our data sharing we have been able to coordinate the Greater Houston Multi Visit Patient Collaborative where Harris Health and Houston Methodist have been able to share data on shared patients to coordinate care and develop individualized care pathways for some of our mutual highest utilizing patients.

Now moving into 2025 we are excited to begin expanding our data sharing with two other of the largest health systems in the Houston area in order to continue building out best practices and serving our patients at the highest level.
