    Harris Health’s Multi Visit Patient Initiative has had a flourishing relationship with PCIC for years now. Through our data sharing we have been able to coordinate the Greater Houston Multi Visit Patient Collaborative where Harris Health and Houston Methodist have been able to share data on shared patients to coordinate care and develop individualized care pathways for some of our mutual highest utilizing patients.

    Now moving into 2025 we are excited to begin expanding our data sharing with two other of the largest health systems in the Houston area in order to continue building out best practices and serving our patients at the highest level. All of this could not be done without the awesome analysts and team at PCIC and we look forward to continued success in the years to come.

    The PCIC database has developed from its initial concept to an operational tool that is invaluable in: 1) community needs assessment, and 2) in examining program outcomes.

    The ability to see across systems is very useful as one gauges the impact of mental illness, substance abuse, medical illness, homelessness, and criminal justice involvement. These data can be persuasive in supporting policy and funding requests. PCIC staff have been capable, persistent, and conscientious in their collaboration with us.

    When we joined PCIC, the overlap data analysis of our client activity in other service systems and estimated costs was staggering. Our clients were falling into a service gap, resulting in costly emergency services that did not address the root cause of their issues.

    Cross-sector data analysis is not only essential; it is critical to see the full scope of service utilization and to identify the community partners necessary to make a case for delivering care differently and improving health outcomes for the person and the community.
