We'd like to welcome our newest PCIC care coordinator, Chris Dammel. Chris is a firefighter with the Houston Fire Department, and has served his community for ten years. He heard about PCIC from current care coordinators, Eric Lobpries and Karen Dupont. Although he's only been with us for a month, he has some great insight about our organization's vision and goals with the clients.

We wanted to share with you our news as we conclude Phase I. The project is moving into Phase II thanks to YOU, our collaborators, stakeholders and funders.
The Patient Care Intervention Center had its stakeholder meeting on September 2nd 2014 , to discuss progress made on the project. We had representation from City of Houston, County Budget Office, County Jail, Greater Houston Healthconnect, Harris Health System, Healthcare for the Homeless-Houston, Houston Recovery Center, and Mental Health and Mental Retardation Authority.
The Patient Care Intervention Center had its kickoff meeting on April 29, 2014. The PCIC board and staff members were joined by representatives of the City of Houston, Harris County Budget Office, Harris County Jail, Harris Health, Healthcare for the Homeless – Houston, Houston Recovery Center, Memorial Hermann Hospital, and Mental Health Mental Retardation Authority.

A new program of identifying patients who most frequently go to Houston-area emergency rooms and then coordinating their medical care has reduced ER visits by 85% in just three months, according to preliminary findings released today by the Primary Care Innovation Center (PCIC).
PCIC research is based on patients identified as "superutilizers" – patients who use emergency rooms across Houston more than 10 times in one year. During the three months before the PCIC program, eight of those patients visited an ER a total of 65 times.
Intensive users of medical services account for 21% of the Medicaid budget in Texas. Four patients used emergency rooms at 19 different Houston hospitals a total of 179 times last year, according to Primary Care Innovation Center, a Houston nonprofit. One patient had a total of 56 visits to 10 different emergency rooms. These intensive users of medical services - in the jargon, "super-utilizers" - account for 21 percent of the Medicaid budget in Texas, according to Primary Care Innovation Center.
