The Patient Care Intervention Center had its stakeholder meeting on September 2nd 2014 , to discuss progress made on the project. We had representation from City of Houston, County Budget Office, County Jail, Greater Houston Healthconnect, Harris Health System, Healthcare for the Homeless-Houston, Houston Recovery Center, and Mental Health and Mental Retardation Authority.

We reviewed the PCIC data portals and dashboards and walked through the business plan as well as the patient selection and intervention workflows. We discussed some of our challenges, as well as potential solutions. We ended the meeting identifying potential resources and next steps for the project.

We would like to thank everyone who participated in the meeting, and look forward to continuing to work together, to make this project a success. A copy of the slides from the meeting can be downloaded from here.
