The Patient Care Intervention Center had its kickoff meeting on April 29, 2014. The PCIC board and staff members were joined by representatives of the City of Houston, Harris County Budget Office, Harris County Jail, Harris Health, Healthcare for the Homeless – Houston, Houston Recovery Center, Memorial Hermann Hospital, and Mental Health Mental Retardation Authority.
Dr. Jeffrey Brenner from the Camden Coalition of Healthcare Providers discussed the fragmentation of the healthcare system and presented the Camden model of targeted and coordinated care as a solution. The meeting’s participants were highly interested in collaborating to adopt this approach in Harris County.
Now a cross-site under the Camden Coalition of Healthcare Providers, PCIC looks forward to working with our stakeholders in improving care and reducing costs for our patients.
