Breaking Down Nutrition Barriers: PCIC and UHCOM Partner to Improve Access to SNAP in Houston’s Third Ward

Healthy meals make a huge impact. In fact, poor nutrition is the main cause of poor health, high healthcare costs, and lower productivity in the US and globally. It leads to health issues like heart disease, cancer, and other serious conditions (Source: American Heart Association).

That’s why Patient Care Intervention Center (PCIC) is partnering with the University of Houston Tilman J. Fertitta Family College of Medicine (UHCOM) in 2024 to break down barriers to nutrition in the Houston Area by improving access to Supplemental Nutrition Assistance Program (SNAP) in Houston’s Third Ward.

Why SNAP Access is Essential for Health Equity

Over 16% of residents of Harris County lack access to enough food for a healthy life (Source: Feeding America).


SNAP provides an essential resource to low-income families who are working hard to make ends meet. Without access to healthy meals, families are more susceptible to serious health problems such as type 2 diabetes and high blood pressure (Source: Feeding America).

Knowledge is Power

The knowledge gained from this collaboration will help the UHCOM project team understand the state of SNAP access in the Third Ward area.

UHCOM knows the impact SNAP can have for families in Houston—but they need data-driven, actionable insights to understand which interventions are most effective to get families the resources they need—that’s where PCIC can help.

Using the powerful Unified Care Continuum Platform (UCCP) technology, our team at PCIC will study diet and nutrition needs related to social determinants of health (SDoH) of Houstonians in the Third Ward before, during and after interventions carried out by community health workers (CHWs).

This collaboration between PCIC and UHCOM follows a successful partnership with Amerigroup to reimagine care for vulnerable populations.

We do this by integrating clinical, behavioral, and social determinants of health into serviceable insights that our partners use to drive action.
