That’s why we’re proud to partner with Houston Food Bank (HFB) to support their Community Assistance Program (CAP) and Medically-Tailored-Meals (MTM) program. Connecting Communities with Resources: Updates on PCIC’s Partnership with Houston Food Bank

At Patient Care Intervention Center (PCIC), we know that health and food security are deeply connected—and addressing both requires cross-sector collaboration. That’s why our partnership with Houston Food Bank continues to grow through two flagship programs: Houston Food Bank’s Community Assistance Program (CAP), now live and expanding, and their upcoming Medically Tailored Meals (MTM) program, launching later this year.

Together, these initiatives are creating scalable, data-driven solutions that improve access to nutritious food, strengthen community health outcomes, and reduce costs for healthcare systems.
